Why This Matters on the Exam

Foodborne Illness: Definitions and Scope
A foodborne illness is any illness or disease transmitted to a person through food. It occurs when a person eats food that has been contaminated by pathogens (biological agents like bacteria, viruses, or parasites), chemicals, or physical objects. Common symptoms include nausea, vomiting, diarrhea, fever, and abdominal cramps.
A foodborne illness outbreak exists when two or more people experience the same illness after eating the same food, and a public health investigation confirms the food as the source. This is a precise definition. One person getting sick from a meal is a foodborne illness. Two or more people getting sick from the same food, confirmed by investigation, is an outbreak.
The distinction matters because outbreaks trigger regulatory action. Regulatory authorities investigate, operations may be shut down, and legal consequences follow. The CDC estimates that roughly 48 million people in the United States experience foodborne illness each year, resulting in approximately 128,000 hospitalizations and 3,000 deaths.

The Five CDC Risk Factors for Foodborne Illness
The Centers for Disease Control and Prevention (CDC) has identified five major risk factors that contribute to most foodborne illness in foodservice operations. These are behaviors and practices, not specific pathogens. Every food safety system you implement should target one or more of these risk factors.
1. Purchasing Food from Unsafe Sources
All food must come from approved, reputable suppliers. An approved supplier is one that has been inspected and meets all applicable local, state, and federal laws. Purchasing from unapproved sources (home kitchens, unlicensed vendors, roadside sellers) introduces unknown risks because the food has not been produced, stored, or transported under controlled conditions.
2. Failing to Cook Food Adequately
Cooking food to its required minimum internal temperature kills most harmful pathogens. Undercooking is one of the most common causes of foodborne illness. Each category of food has a specific minimum internal temperature it must reach (covered in detail in Chapter 6).
3. Holding Food at Improper Temperatures
Food held in the Danger Zone for too long allows pathogens to multiply to dangerous levels. Hot food must be held at 135F (57C) or higher. Cold food must be held at 41F (5C) or lower. This risk factor also includes improper cooling and reheating.
4. Using Contaminated Equipment
Cross-contamination occurs when pathogens transfer from one surface or food to another. This commonly happens through cutting boards, utensils, and equipment that contact raw food and then touch ready-to-eat food without proper cleaning and sanitizing in between.
5. Practicing Poor Personal Hygiene
Foodhandlers who do not wash their hands properly, work while ill, or touch ready-to-eat food with bare hands can transmit pathogens directly to food. Handwashing, proper glove use, and illness reporting policies address this risk factor.

TCS Foods: Time/Temperature Control for Safety
TCS food (Time/Temperature Control for Safety food) is any food that requires time and temperature control to limit the growth of pathogens or the production of toxins. These foods are high in protein, moisture, or have a neutral to slightly acidic pH, all conditions that support bacterial growth.
You must be able to identify TCS foods because they drive nearly every food safety procedure: cooking temperatures, holding requirements, cooling methods, and reheating protocols all apply specifically to TCS foods.

Why These Foods Are Dangerous
TCS foods share characteristics that bacteria need to thrive. The acronym FAT TOM describes the six conditions that support bacterial growth:
- Food - Bacteria need nutrients, especially proteins and carbohydrates
- Acidity - Most bacteria grow best in food with a pH between 4.6 and 7.5
- Temperature - Bacteria grow most rapidly between 41F and 135F (5C and 57C)
- Time - Given the right conditions, bacteria can double in number every 20 minutes
- Oxygen - Some bacteria need oxygen; others grow without it
- Moisture - Bacteria need moisture to grow. The water activity level that supports growth is 0.85 or higher
TCS foods provide favorable conditions across most or all of these factors. That is why they require strict time and temperature controls that non-TCS foods do not.
Exam Alert: Cut melons, cut tomatoes, garlic-in-oil mixtures, and sprouts are TCS foods that are frequently tested because candidates often forget them. Memorize the full list.
Exam Alert: Baked potatoes are a TCS food. A whole raw potato is not. The act of cooking changes the potato's characteristics and makes it capable of supporting bacterial growth under the right conditions.
Scenario: Identifying TCS Foods
A caterer is preparing a buffet with the following items: whole fresh apples, sliced cantaloupe, cooked rice pilaf, a basket of dinner rolls, a tray of grilled chicken, and a bowl of dry crackers.
Which items are TCS foods that must be temperature-controlled?
- Sliced cantaloupe (cut melon)
- Cooked rice pilaf (cooked grain)
- Grilled chicken (poultry)
The whole apples, dinner rolls, and dry crackers are not TCS foods because they lack the moisture and conditions needed for rapid bacterial growth.

The Danger Zone
The Danger Zone is the temperature range between 41F and 135F (5C and 57C). Within this range, bacteria that cause foodborne illness can grow rapidly. The most dangerous part of the Danger Zone is between 70F and 125F (21C and 52C), where bacteria multiply most quickly.

Time-Temperature Abuse
Time-temperature abuse occurs any time food is allowed to remain in the Danger Zone longer than permitted. This happens in three common ways:
- Food is not cooked to the required minimum internal temperature
- Food is not held at the correct temperature (hot or cold)
- Food is not cooled or reheated correctly
Every minute a TCS food spends in the Danger Zone, bacteria are multiplying. After 4 hours total time in the Danger Zone, the food must be discarded. Some operations use a time as a public health control method, which allows food to remain unrefrigerated for a limited period under strict monitoring (covered in Chapters 6 and 7).

Exam Alert: Freezing does not kill bacteria. It only stops bacterial growth. When food thaws, surviving bacteria resume multiplying.
High-Risk Populations
Certain groups of people are far more likely to become seriously ill, be hospitalized, or die from foodborne illness. These are called high-risk populations (also known as highly susceptible populations). Food prepared for these groups requires stricter controls.
The four high-risk populations are:
- Elderly individuals - Immune systems weaken with age, and organs become less effective at filtering pathogens
- Children under 5 years old - Their immune systems are not yet fully developed
- Pregnant women - Pregnancy suppresses certain immune responses, and some pathogens (like Listeria) can cause miscarriage or stillbirth
- Immunocompromised individuals - People with HIV/AIDS, cancer patients undergoing chemotherapy, organ transplant recipients on anti-rejection drugs, or anyone with a condition that weakens the immune system
Operations Serving High-Risk Populations
Facilities that primarily serve high-risk populations include:
- Hospitals and nursing homes
- Preschools and daycare centers
- Assisted living facilities
- Meal delivery programs for homebound elderly
These operations face additional restrictions. For example, they typically cannot serve undercooked eggs, raw seed sprouts, rare or medium-rare meat, or unpasteurized juice. The standards are higher because the consequences of a foodborne illness in these settings are more severe.
Exam Alert: The exam may present a scenario set in a hospital, daycare, or nursing home and ask what food items should NOT be served. Know that raw sprouts, undercooked animal products, and unpasteurized items are prohibited for high-risk populations.
